OUR NURSE CARE COORDINATION & SKILLED NURSING SERVICES Every Carelink client has a Registered Nurse Care Manager. The benefit of this service includes:
Sets up and oversees the in-home care system to meet individual needs
Coordinates care with physicians, family and other healthcare providers involved in the care plan
Performs home safety assessments
Provides training, supervision, guidance and support to caregivers
Provides client and family education on care and management of chronic medical conditions
Completes an initial nursing assessment and visits every 45 days and as needed
Reduces hospitalizations and Emergency Room visits
Develops a personalized service care plan to meet the client’s needs
